If you are participating in ANY scuba/snorkeling training course and have answered "YES" to any of the medical questionnaire questions, we must insist that you submit a copy of the following "Diver's RSTC Medical Statement" to your doctor for approval. This must be completed and submitted to your instructor, before you continue your training with Underwater Phantaseas or any other facility.
